Bolton NHS Foundation Trust’s specialist homeless and vulnerable adult and diabetes teams have been shortlisted for two awards at the Diabetes Nursing Awards 2024.
Led by Diabetes Specialist Nurse, Lynne Wooff, Bolton’s teams have been shortlisted for Service Improvement Project of the Year and the Innovation Award.
Lynne said:
It’s an honour to have been shortlisted for two awards and for us to have been recognised for all the work we do to support those that are homeless to better manage their diabetes and to also carry out diabetes screening in this vulnerable cohort.
The entire team work holistically with people, teams and groups throughout Bolton’s communities so that we can support those who need our help, and to improve their outcomes from diabetes related complications.
