The Homeless and Vulnerable Adults Service aims to meet the needs of the homeless and vulnerable adults living with complex lifestyles within the Bolton area.

We provide outreach services to hostels, on the street and temporary accommodation, in-reach to Royal Bolton Hospital and drop-in clinics at Lever Chambers Health Centre (Monday, Wednesday and Friday 10am – 2pm) for those with no GP and with health problems associated with being homeless or having drug and / or alcohol dependencies and vulnerably housed.

Our aim is to provide a complete health assessment of a patient’s immediate health needs, access to GP registration and to support patients in accessing health, social and housing services.
