The Neurology Long Term Conditions Team provides a service for adults with long term neurological conditions such as, multiple sclerosis, Parkinson’s disease, epilepsy and motor neurone disease, as well as acquired neurological conditions such as spinal cord injury and head injury.
We work together to treat and support patients with neurological long term conditions and their carers. We work to achieve agreed goals, with the aim of maximising potential and participation in the local community.
Our team is closely linked with regional specialist centres, local health and social care providers and with local voluntary groups.

Patients can be referred to our team by any relevant health or social care professionals. We also accept self-referrals for patients who have been known to the service previously. Referrals are then triaged by an appropriate clinician and appointments allocated according to the patient’s needs.
Typically, we see patients in clinic rooms at the health centre, in their own homes, in the local community or in the therapy gym at Breightmet Health Centre. It very much depends on the patient needs and the most appropriate environment to meet clinical needs.
